A Note from the UCAN Director
This message about the state’s immunization requirement is extremely important!
Nevada state law
requires that ALL students who attend a University credit course anywhere
in Washoe County MUST provide proof of immunization before class begins.
Missing documentation prevents you from registering and receiving credit.
The University is required to collect this documentation.
Section 1. NAC 441A.755 1. . . unless excused because of religious belief or medical condition, a person shall not attend a university until he submits to the university proof of immunity to tetanus, diphtheria, measles, mumps, rubella and any other disease specified by the State Board of Health.
Section 1. NAC 441A.755 9. Any student who is enrolled in a program of distance education and who does not attend a class on campus is exempt from the requirements of this section. [“On campus” is defined by the University as all of Washoe County.]
Please submit a copy of official documentation for two doses (one month apart) of measles, mumps, rubella (MMR) and a diphtheria/tetanus (DT) within the past 10 years. If you were born prior to 1957, you only need to provide documentation of the DT shot.
Please send documentation to:
Extended Studies/048, University of Nevada, Reno, NV 89557. Or fax to: (775) 784-4801. Be sure your name and address are shown clearly on the document.
I’m sorry about this, but everyone MUST comply.
Questions? Please give us a call at (775) 784-4046 or 1-800-233-8928. We’re here to help. We want you to be able to access the classes you need in as smooth a process as possible.
Sincerely,
